GLP-1 and Semaglutide Meds – The Pros, Cons, Unknowns and How To Safely Use Them

GLP-1 (Glucagon-Like Peptide-1 receptor agonists) and the semaglutide medications are all the rage and with that have become the center of conversations around their safety and efficacy.  These medications that began by being prescribed for managing diabetes, have gained popularity for their additional benefits, such as aiding in weight loss. However, like any drug, they come with their own set of pros, cons and unknowns. With so many of my clients beginning to use these as a way to help with their weight loss struggles, I’ve decided it’s time to address some of the benefits, downfalls and unknowns as well as how to best use them for safe and effective outcomes.

What is GLP-1?

GLP-1 and semaglutide medications work by increasing the levels of the GLP-1 hormone in the body. When GLP-1 levels are increased, it stimulates the pancreas to release more insulin in response to eating, which helps lower blood sugar levels. It also reduces the amount of sugar the liver produces, slows down the emptying of the stomach, and makes you feel full sooner, which can help with weight loss. This combined effect helps manage blood sugar levels and can aid in weight reduction, making these drugs effective for treating type 2 diabetes and obesity. Both GLP-1 medications and semaglutide are peptides. Peptides are short chains of amino acids, and GLP-1 (Glucagon-Like Peptide-1) itself is a natural hormone peptide. The medications are designed to mimic or enhance the action of this peptide hormone in the body to help regulate blood sugar levels and control appetite.

The peptides were first used to help Type 1 diabetics with blood glucose control and the side effect was weight loss through their work in the liver, pancreas and gut.  Seeing the side effect of weight loss, the drug companies jumped on the opportunity to market this as a weight loss remedy.  These peptides are not new and have been around for many years being studied since the 1970’s and being used for diabetics in the early 2000’s.  Research over the past 40 years on GLP-1 has been extensive and we have learned it often interacts with several body organs and may have other benefits in the heart as well as the brain. When we use a peptide to increase a natural compound in the body this is often a very good thing however, if we abuse it and use too much it can become problematic.  This is where we find ourselves today in the controversy of using these medications as many doctors are using much higher doses than are safe or even required for healthy weight loss.

Pros of GLP-1 Medications

Obesity and diabetes are growing problems in our country and are associated with an increased risk of heart disease, stroke, high blood pressure, certain cancers, and a reduced quality of life.  Just about everyone it seems struggles with weight and weight loss diets often fail due to the inability of people to stick with them as a lifestyle.  So let’s look at the pros of these medications as you consider whether they are right for you.

1. Blood Sugar Control: These medications help manage blood sugar levels by increasing insulin production and reducing glucose release from the liver, which is great for those with type 2 diabetes or insulin resistance.

2. Weight Loss: Many people experience weight loss while taking GLP-1 medications, which can improve insulin sensitivity and overall health.

3. Heart Health: GLP-1 medications may reduce the risk of heart disease by improving heart function and lowering blood pressure.

4. Reduced Appetite: These medications can help curb your appetite, making it easier to stick to a healthy eating plan and avoid overeating.

5. Brain Health: There’s promising research suggesting that GLP-1 medications might help reduce the risk of neurodegenerative diseases.

Cons of GLP-1 Medications

With so many promising effects from these injections, why wouldn’t everyone want to use them?  After all, don’t most people want to lose weight, lower insulin levels, improve brain health and decrease their risk of cardiovascular disease?  Remember, that there has never been a magic pill for weight loss and everything has a cost.  Let’s take a look at the downsides.

1. Side Effects: Common side effects include nausea, vomiting, diarrhea, and abdominal pain. These symptoms can be mild to severe and may require adjustments in dosage or discontinuation of the medication. You can also take cannabis for ibs to mitigate the symptoms. Less common but more severe side effects include pancreatitis, gastroparesis, bowel obstruction, gallstone attacks, and bile duct blockage.

2. Cost: GP1 medications can be expensive, and not all insurance plans cover them. This can be a significant barrier for many people.  Keep in mind that it is recommended for people to stay on these injections for life.  Studies have shown that people will regain their weight if they come off them completely unless they have changed their lifestyle behaviors.

3. Risk of Hypoglycemia: Although less common with GP1 medications compared to other diabetes treatments, there is still a risk of low blood sugar, especially when combined with other medications.

4. Loss of Muscle: While all diets will result in some lose of muscle, it seems that the severe calorie restriction that comes along with usage of this injection can cause excessive muscle loss and if weight is regained it will most likely be fat, not muscle.

The Unknowns of GLP-1 Medication

Here’s where the real issue comes and that is that we do not know the long term side effects of these high doses of medications that are being used.  Keep in mind that if a doctor was using these at a low dose, just to restore your bodies natural hormone, I doubt you would see any of the side effects or have long term issues.  So far the only studies have been in mice and they did see a correlation with some thyroid cancer, pancreatic inflammation, altered kidney function, gut disturbances and cardiovascular issues. The long-term effects of GP1 medications are still being studied. While current research is promising, it’s important to note that we do not yet fully understand the potential risks of prolonged use.

Safely Taking GLP-1 Medications

To maximize the benefits and minimize the risks, consider the following tips for taking GLP-1 medications safely:

1. Stick with a Low Dose: Start with a low dose and stick with a low dose.  Be patient!  Higher doses of this peptide is where we are most likely going to see the downsides while a nice low dose is going to act like it is supposed to naturally.  Consider this:  Insulin is a peptide and we wouldn’t up someones insulin dose for a better result because it would have an adverse affect on their health.

2. Aim for Slow, Steady Weight Loss: Rapid weight loss can lead to muscle loss and nutritional deficiencies. Aim for a gradual weight loss of 1-2 pounds per week, which is more sustainable and healthier for your body.

3. Incorporate Weight Lifting with Substantial Weight: Adding weight lifting to your exercise routine can help preserve muscle mass and boost metabolism. Muscle tissue burns more calories than fat, even at rest, which can aid in weight management. Using substantial weights can further help prevent muscle loss, making weight training a crucial aspect for anyone considering using these drugs for assistance.

4. Get Enough Protein: Protein is essential for muscle repair and growth, especially when losing weight. Ensure you are consuming enough protein through lean meats, fish, beans, nuts, and dairy products.  I recommend at least 1 gram per pound of ideal body weight for anyone on a weight loss program or all adults over 50.

5. Work with a Nutritionist: A nutritionist can help ensure you get enough protein and other essential nutrients. For long-term weight loss, it’s crucial to learn how to eat and change eating habits. 6. Cycle On and Off: Consider cycling on and off GLP-1 medications and keeping the dose low for slow and steady weight loss. This approach can help your body adjust and may reduce the risk of side effects.  Doctors are told this medication is for life but like any peptide it should be cycled for best results.

7. Monitor Your Blood Sugar: Regularly check your blood sugar levels to ensure they are within the target range. This can help you and your doctor make necessary adjustments to your medication and lifestyle.

8. Stay Hydrated: Drinking plenty of water is crucial, especially if you experience gastrointestinal side effects. Staying hydrated can help alleviate some of these symptoms.

9. Consult Your Doctor Regularly: Regular check-ups with your healthcare provider are essential. They can monitor your progress, adjust your medication as needed, and address any concerns or side effects you may experience.  Consider a doctor who can measure your body fat and muscle each visit so you can be assured you are losing mostly fat.

In Summary

GLP-1 medications can be helpful for someone who is struggling to lose weight or has weight loss resistance, however they are not a replacement for changing behaviors.  Patients that choose to use these medications at high doses to lose weight rapidly will more than likely pay the price dearly with other health issues and weight gain if they discontinue the peptide. I do think there is a place for them but I plead with you to be aware of the consequences should you choose to stick with old eating habits, ignore weight lifting or lose weight too rapidly.
